Your Medical Records

The medical records department for Clear lake Endocrinology can be reached at (281) 554-4075.

Medical records are kept confidential. We will not release any of your medical information without first obtaining a signed authorization form from you.

We do not charge for copying of medical records if they are going directly from this office to another physician's office. We do charge if you are picking up a copy of the records yourself or if the records are going to a 3rd party such as a lawyer's office, insurance company, etc. then the fees are as follows:

• For pages 1-10 - No Fee.
• For pages 11-49, a fee of $0.50 cents per page.
• For pages 50 or greater, $0.25 cents per page.

This procedure abides by the state law requirements for Texas.

In addition, and as required by federal HIPAA law, all patients that desire to transfer their medical records are required to sign an Authorization For Release of Protected Health Information prior to the release of any records.

Our turn-around time is 7-10 days for the copying of medical records.
